Recertification Eligibility Requirements - Board Certification

Applicants must be physicians who, at the time of application, hold one of the following certifications:

Cardiovascular Disease – Board certified in Cardiovascular Disease by the American Board of Internal Medicine or subspecialty certificate in Cardiology from the American Osteopathic Board of Internal Medicine

Nuclear Medicine – Board certified in Nuclear Medicine by the American Board of Nuclear Medicine or the American Osteopathic Board of Nuclear Medicine

Radiology – Board certified in Radiology by the American Board of Radiology or the American Osteopathic Board of Radiology

Alternate Pathway – If you originally certified under the Alternate Pathway and met the board certification requirement in an area other than Cardiovascular Disease, Nuclear Medicine or Radiology, you must provide a statement attesting to this along with documentation of your current board certification.

Training Outside the U.S. – If your Cardiology, Nuclear Medicine or Radiology training was completed outside the U.S. you are required to submit evidence of certification from an equivalent medical specialty board in your country of practice accompanied by the name of the certifying organization

Documentation to Verify Board Certification
One of the following documents verifying board certification must be submitted with the application:

  • A letter from the certifying board verifying passing of the board exam including the effective and expiration dates
  • A certificate from the certifying board showing the effective and expiration dates
  • A print-out from the certifying board’s website showing the effective and expiration dates
  • For non-U.S. trained physicians, a copy of equivalent documentation verifying training credentials and/or national registration as specialist in Cardiology, Nuclear Medicine or Radiology.
